The Future Planning Template has been designed by local clinicians to help GPs record Treatment Escalation Planning, End of Life and Avoiding Unplanned Admissions information in the patient record. With patient consent, all of this information is then available to view immediately by ambulance services, Out of Hours GP services, emergency departments, medical assessment units and community teams in the patients Summary Care Record (SCR).
Instructions below show how this SCR information can be accessed in either RiO or SystmONE. In all cases when accessing SCR information patient consent must be considered. However, if you require access to the information as a clinician providing care to a patient and the patient lacks capacity, or has given implied consent by agreeing to referral to a specialist team, or through not accessing the information you potentially delay or reduce the quality of care that you provide, then consider accessing without consent. Both systems give the opportunity for you to record your reason for accessing a patient's SCR. A reason should be recorded as this aids audit and protects your back.

SystmONE access to Summary Care Records
1. Access the patient record in SystmONEThe Summary Care Record for a patient is found, as shown above, at the bottom of their "Access Views" folder. This may vary in different trusts/practices - if not sure ask IT helpdesk.

2. When opening the SCR, select the appropriate consent option.See the notes above and enter your reason for accessing the record, especially if accessing without consent.

RiO access to Summary Care Records
1. Find the patient's Summary Care Record at the bottom of the Case Record Menu.
For ease of access in the future you can drag and drop the Summary Care Record link to higher in the case record menu. This will then show in the new position when you access any other patient's record.
Press and hold down the left mouse button while over the Summary Care Record icon or words. Then, keeping the left button pressed, drag the link higher up the menu. Release the button to drop the link into place wherever you want. Feel free to repeat if the link lands in the wrong spot.
