When patients come into hospital, they may already have a Future Care Plan in place. When a Future Care Plan is created the patient and/or family are given a hard copy and could have brought this with them. However, if this is not the case then many Future Care-plans can often be found in the Care Plan/Care Plan section on the Care and Health Information Exchange. If Future Planning conversations are held whilst an inpatient or in an Outpatient setting, please pass these details onto the Primary Care team and Community team colleagues.
