It is also possible to download an electronic version of the My Wishes leaflet via this website from the Dodgy Handwriting page.
If any Future Planning information is captured using the My Wishes leaflet then this needs to be passed to the GP practice. The practice should add this information to the coded Future Planning template ensuring it is visible in the patients national Summary Care Record and that a Future Care Plan is printed and saved to CHIE. This means out of hours services can access the information whenever and wherever needed.
