MiQuest V2- Prognosis ordered Practice Register

We have been told that all GP practices know how to download and install templates, self populating printable documents and run MiQuest reports. If this is not the case, then please refer to the guides on our download pages.

The Future Planning MiQuest report generator, produces a list of all patients in the practice who have a prognosis code entered in the Future Planning template. The list if printed in colour shows all patients for your GSF, Frailty or Long Term Conditions meetings in prognosis order. 

All instructions for producing the MiQuest report can be found in the downloadable excel file.

Unfortunately with the arrival in 2019 of SNOMED CT coding in GP systems, the NHS has decided to no longer support MiQuest. Over the next few months we will be seeking ways of producing the same Prognosis Ordered Practice Lists via EMISweb and SystmONE. For the moment though MiQuest continues to limp on.

Some CCGs are due to gain the ability to distribute new templates and self-populating documents directly into the clinical systems of CCGs practices. Where possible we will use this route for updates to the Future Planning Template.

Future Planning Care Plan - self populating printable document

The Future Planning Care Plan Self populating report is run from within S1 or EmisWEB and works in an identical way to self populating referral forms.

Future Planning Template Coding document

​This documents all of the READ2 and CTV3 codes used in the Future Planning Template. New national SNOMED CT codes were requested by the project and released in April 2017 to allow a move to a fully SNOMED CT coded Future Planning template in late 2017.

Upload to the Summary Care Record

With patient consent all codes in the Future Planning Template automatically upload to the Summary Care Record (SCR).
We are currently using three CTV3 and six READ 2 codes that are not part of the SCR Additional Information coding set. In order for these three codes to be raised to the SCR we add them to the Patient Summary Report in SystmONE and classify them as "problems" in EMISweb. Apologies if this makes the patients Summary View or Problem List look a little strange, however until additional national codes are available, the only way to get important information such as "Patient requires referral to the coroner", "Patient wishes to be a tissue donor" or "Patient is at risk of CO2 retention" up to the SCR is to apply this work around.

Thanks to the National SCR team, who added a number of extra codes to the SCR Additional Information dataset in April 2017.

We hope that in 2021 all of the information added to the Future Planning template will also be viewable in a new Urgent Care view in CHIE (Hampshire and Isle of Wight Clinical Health Information Exchange). Future Planning information visible in the Urgent Care View will be one of the first clinical applications of the upgraded CHIE (using Orion software).
