My Wishes leaflets should be present in most GP reception areas and can be ordered from Harlow Printing or in some areas from your comms team or ordering system. Use them and the AgeUK "Let's talk about death and dying" booklets to help patients and families to consider their Wishes for care in the future. In many cases patients will express their wishes during community care team visits allowing you to enter this information directly into the Future Planning template in Community/Hospice SystmONE.

If a patient has already completed a leaflet and handed this to their GP reception, then their information should be visible in their Summary Care Record, in the HIoW Healthcare S1 Future Planning template, FP view or Future Care Plan printout.

Once in the patient record in SystmONE look for the Future Planning template and Future Planning Quicklinks templates in the left hand tree.
