The Future Planning Template is a communication tool that sits in your GP clinical system and is specifically designed to help improve safe sharing of important summary information from your clinical record. This is important for anyone living with one or more chronic medical conditions, an End of Life diagnosis, frailty or a mix of these. All of these conditions may lead to sudden deteriorations and most of these can be anticipated and planned for. Previously clinical teams caring for you in a crisis would have had very little, if any, medical information about you. The Future Planning template helps to transfer a summary of your medical record to the National Summary Care Record (covering all of England), which can then be securely read by clinical teams providing your urgent emergency care. These services include: out-of-hours doctors, the local ambulance service, community nurses, hospital staff and specialist teams relevant to your medical condition. Only people who directly care for you are allowed to access and read your record.
