The Future Planning Template has been designed by local clinicians to help GPs record Treatment Escalation Planning, End of Life and Avoiding Unplanned Admissions information in the patient record. All of this information is then available to view immediately by ambulance services, Out of Hours GP services, emergency departments, medical assessment units and community teams in the patients national Summary Care Record (SCR).
March 2023 has seen the addition of a Future Planning form in RiO community in South East Hampshire, initially, but perhaps next year across other community areas using RiO in England.
April 2023 will see widening of access to the Future Planning Template in SystmONE community in Portsmouth.
Both of these rollouts are accompanied by eLearning, a new staff video narrated by Hugh Bonneville, patient and clinic resources and ICB (Hampshire & Isle of Wight Integrated Care Board) backing.
Instructions in this community teams sectionshow how this SCR information can be accessed in either RiO or SystmONE. In all cases when accessing SCR information patient consent must be considered.
Should you need to access these records without patient consent then the SCR gives the opportunity for you to record your reason/s. Whether due to a patient lacking capacity, seeking additional information as part of a referral (particularly where a Local Care Record is not available) or as part of an urgent or emergency response, please briefly record the circumstances as this aids audit and protects your back.
