We want to help make sure that when you become ill that your wishes and how you would like to be cared for, are known, visible in your medical record and are acted upon. The Future Planning Template is a tool that sits in your GPs computer system and helps to improve safe sharing of important information from your GP record. This is important for anyone with one or more long term medical illnesses, an End of Life diagnosis, frailty or a mix of any of these. Most illnesses can suddenly worsen with a person becoming unwell and most of these can be planned for. In the past, clinical teams caring for you in a crisis would have had very little, if any, medical information about you. The Future Planning Template helps to get a summary of your GP record into the National Summary Care Record (covering all of England). This can then be easily and securely read by clinical teams providing your care in a medical emergency. These teams include: out-of-hours doctors, the local ambulance service, community nurses, hospital staff and specialists in managing your condition. Only people who care for you are allowed to access and read your record.
