Looking for information in the Summary Care Record from within SystmONE is only likely to be needed for patients who have moved from a GP practice that uses EMIS or Vision.

1. Access the patient record in SystmONEThe Summary Care Record for a patient is found, as shown above, at the bottom of the "Access Views" folder. This may vary in different trusts/practices - if not sure ask IT helpdesk.

2. When opening the SCR, select the appropriate consent option.See the notes on the Community Teams page and enter your reason for accessing the record, especially if accessing without consent.

3. Scroll to the bottom of the patient's SCR to find the headers shown.Information from the GP Future Planning Template will mostly be shown in these sections of the SCR.
