Guidance to support the move from Oral to SubCutaneous End of Life Medications
Identical EMIS and SystmONE templates, shown at the bottom of this page, were installed in general practice clinical IT systems across Hampshire, Southampton and Portsmouth in April 2020 alongside the Future Planning Template Version 3.3. Both resources aim to support consistent and safe community prescribing for End of Life patients.
In addition, all General Practices supplied with EMIS templates by Ardens Q across England also gained access to this free resource in April 2020.
