This page shows the two ways to use the "My Wishes" leaflet. Whether you like writing it out by hand or completing the online form, we're here to guide you through them both.

First, a handwritten My Wishes form for handing into Ellie's GP Practice Reception.
Once the GP practice admin staff have added Ellie's information to her GP record, and possibly after a chat with her GP, Ellie should be given a printed Future Care Plan, Front sheet and Fridge Magnet by her practice.   

Second, a typed My Wishes word document for: -

• Printing, signing and posting or handing in to Mr Example's GP practice.

 OR probably easier

•  Saving the word document and emailing to his practice.                                         

Once the GP practice admin staff have added his wishes to his GP record, and maybe with a bit of clinical advice from his GP, he should also be given a printed paper Future Care Plan, colour Front sheet and Fridge Magnet by his practice.
