In order to track the use of the Future Planning Template anonymous information was collected each quarter between 2017 and 2021 by the Hampshire Health Record informatics team. These reports show figures for each CCG, with anonymised general practices and any counts of less than 5 redacted to ensure complete practice and patient confidentiality. Individual CCGs were able to receive reports with practices in their area identified, however the HHRA Information Governance team only provided these figures to a CCG once they were assured that practices had consented to this identification. Neither report produced any patient identifiable data. With the move of the Hampshire Shared Care Record CHIE from Graphnet to Orion software, it was decided to remove the anonymised database function. We are therefore in discussion with the new HIoW ICB with the aim of using population health and other ICS informatics resources to re-instate similar reports. We should then be in a position to continue to track the increasing collection of Advance Care Planning information across the HIoW Integrated Care System both for Integrated Care Board, but also for Primary Care Networks to support the Frailty Anticipatory Care function and DES reporting requirement.

For practices already using alternative End of Life or Avoiding Unplanned Admissions templates, do not feel you have to move to using our template. Any EoL or AUA coded information you add to your templates will also be counted by the report. We just hope that you think the Future Planning Template is better, and that you switch to using it to make your very busy lives a little bit easier!

The impact of COVID 19 and the need for Treatment Escalation Planning
Hopefully we will never see a graph like this in the future. With deployment of the Future Planning template V 3.3 to every practice across Hampshire in April 2020, to support recording of patient wishes and preferences, it proves many more clinicians can engage with Advance Care Planning than may have done so in the past.

To be fair figures will need to be expressed as activity rate per 1000 practice population, which our anonymisation doesn't allow. Reports are anonymised for the Future Planning team, however your CCG can see who you are!
